Housed within the Northwell Health System. Provides TGNB Specific care, including Primary care, hormone therapy, HIV testing, Mental Health Assesments, Behavioral Healthcare, and case management

Services include Primary medical care, HIV treatment and prevention (i.e. Pre-exposure prophylaxis or PrEP), Hepatitis C services, Opiate dependence treatment (Suboxone), Mental Health (psychiatric care and psychotherapy), and LGBT Health services

Services for people living with HIV/AIDS, LGBTQ+, including HIV Testing and prevention education, case management, Behavioral Health Services and assistance and programs for the uninsured.

Clinical care provided for West Harlem and Washington Heights. Primary Care is available in addtion to Specialty Services. Specialty Services include: Bariatric Services, Cancer, Dermatology, Digestive Diseases- Gastroenterology, Emergency Services, Endocrinology and Diabetes, Geriatrics...
